How Does it Work?
You will receive an email and/or text message to complete patient intake forms prior to your scheduled visit. The email will have details on how to access the link to your virtual visit. You will be able to access your appointment from anywhere that you choose via mobile device or computer.

Complete Required Patient Intake Form
Via the e-mail provided upon registration, you will receive a link to access your intake forms. These are required before any Visits are completed.
Medical History Evaluation by Provider
Your medical history and intake forms will be thoroughly reviewed by one of our Licensed Providers and if no additional information is required from you and no contraindications to treatment are identified.
- Acute illness includes medication if applicable
- Medication Refill
- Work Note/Excuse
